Rinjani Crater Rim with Syam Trekker: Six Max, Toilet Tent, Boat to the Gilis

What comes with it: an experienced guide and a porter, pick-up and drop-off anywhere on Lombok, and, at the end, a transfer by private boat to Gili Trawangan, Meno or Air. That last one matters if the Gilis are your next stop, because it takes the public ferry out of the plan. Food and drinks on the trek are included, and so is a waterfall visit on the day you arrive. The camping kit is the part worth reading: tent, sleeping bags, mattresses 5 cm thick, a toilet tent, camping chairs and a table, and trekking poles.
